CDCFIB — Requirements for Physical Screening & Document Verification

All applicants invited for physical screening and document verification must attend with the original documents

 listed below. Where applicable, also bring clear photocopies.

REQUIRED DOCUMENTS & DETAILS/EXAMPLES:

1. Recent passport photograph  

   Passport-size photo with a plain white background

2. Educational certificates  

   SSCE, ND, NCE, HND, Bachelor's degree, etc.

3. Birth certificate / Age declaration  

   Birth certificate or declaration of age issued by NPC, Court, or recognized government hospital

4. Letter of indigeneity 

   Issued by state or origin certificate

5. Medical fitness report 

   Issued by a government hospital

6. National Identity Number (NIN)  

   Provide NIN slip or valid NIN documentation

7. Bank Verification Number (BVN)  

   Provide BVN slip or bank confirmation

8. Acknowledgement slip  

    Receipt/acknowledgement from the recruitment portal

9. Letter of good conduct  

    Issued by area head or district authority

10. Guarantor’s form

   Completed guarantor information (as required by CDCFIB)

11. Examination slip  

   Original exam/test slip provided during registration

IMPORTANT:

Ensure all original documents are valid and legible. Bring photocopies of each document for submission.  

Failure to present the required documents may disqualify you from further screening.
